Children in Care Leaving Care (Transitions) Team

The Leaving Care (Transitions Team) assists Looked After young people through the process of Leaving Care and provides aftercare advice, support and financial assistance both to Trafford care leavers and also, on behalf of other Authorities, to care leavers from other areas who come to live in Trafford.  The Service also provides advice and support to homeless 16 and 17 year old young people in need and to 16 and 17 year old unaccompanied asylum seekers who are living independently.  The team is made up of Personal Advisors(PA’s) who will work directly with young people to assess their needs and agree any support they may need through a Pathway Plan which is completed when young reach 15 and a half yrs.  Also in the team we have a Connexions worker who is on hand to advise and support young people with education , employment and training issues.  We also have established links with emergency housing in Trafford(HOST) and specialist Community Nurses to ensure that young people keep healthy.
